We will verify your insurance plan and confirm your appointment.
We will then send you the link for your secure online client portal where you can complete all your initial appointment documents, upload your insurance card, and be able to securely communicate with us.
Other Options
If your insurance doesn’t cover our services, you reach your coverage limit, or you just wish to pay out-of-pocket, we can also provide our pricing outline with our out-of-pocket costs for services. We provide individual and group pricing plans for consults, multiple visit packed pricing, and group program pricing plans. Please contact us and one of our representatives will reach out to discuss our services and provide you with our pricing outline.
Clients with a Health Savings Account (HSA) or a Flexible Savings Account (FSA) may be able to use them to cover our nutrition visits. Because we don’t have access to those plans, however, we ask you to contact your HSA/FSA to determine if nutrition consults with us will be covered. Those debit or credit card accounts can be used for payment for each visit. Further details are explained in our Policy and Payment Form required for each visit.
If we are not an in-network provider for your plan, we will provide you with a reimbursement form (superbill) for each visit. This will contain all the necessary procedures and diagnostic codes for reimbursement. We will also provide you with our Services Rate List to help you decide if you’d like to purchase a package (at a discounted rate), or individual sessions, which are charged at the time of your appointment.
